Directions for Ordering Transcripts

OFFICIAL TRANSCRIPT

An official transcript is an official record of all academic work taken at MidAmerica Nazarene University and lists any degrees earned from MNU. The official transcript is printed on blue security paper, bears the official seal of the university and the Registrar's signature. In order to request an official transcript, the requestor must not have any outstanding obligations to the school.

To order a transcript:

1. Print, fill out the Official Transcript Request Form (PDF 623 KB Help ) and mail to:

   Registrar's Office
   2030 E College Way
   Olathe, KS 66062.
   

   You may also fax the completed form to 913-971-3414. The fee for an official transcript is $4.00 per transcript. If sending request via fax, you must include a credit card number for payment (VISA, Master Card or Discover.) Include credit card number, name printed on the card, and the expiration date.
   OR
2. Send a written request, either through the mail or by fax, which includes the following:
   1. Your signature
   2. Your social security number (for identification purposes)
   3. Your date of birth
   4. Your current address
   5. Address you want the transcript sent to if different than your home address.
   6. Phone number
   7. E-mail address
   DO NOT SEND THE TRANSCRIPT REQUEST VIA EMAIL - WE MUST HAVE YOUR SIGNATURE!!!

UNOFFICIAL TRANSCRIPT
An unofficial transcript is an unofficial record of all academic work taken at MidAmerica Nazarene University and lists any degrees earned from MNU. The unofficial transcript is printed on plain white paper, does not bear the school's seal or the registrar's signature. To order an unofficial transcript, include the same information as listed for the official except there is no fee for the unofficial transcript, if you want it mailed to you.
